Medical Form

Release of Liability

All campers--adult, youth and children--must complete a medical form and release of liability form and bring it to camp. Only one form PER FAMILY is required. All the information will be kept confidential and used only in case of an emergency. Children and youth not accompanied by a parent MUST bring a completed form signed by a parent.
