Release of Liability
Release of liability form is REQUIRED before registration is complete.

Medical Form
All campers must have a medical form on file at camp. Only one form PER FAMILY is required. All the information will be kept confidential and used only in case of an emergency. Children and youth (under age 18) not accompanied by a parent MUST provide a completed form signed by a parent.
